Remember that anyone over the age of 18 must call to make his or her own appointment. Be sure to have your insurance card available when you call so that our clerical staff will be able to get all the necessary information to set up your appointment. You can reach our office at 740-689-6700.

To provide greater convenience to current or future clients who live in or around the Pickerington area, our new office will be open beginning Jan. 3, 2011.
