Founded in 1978, The Meadowbrook Care Center embraces a holistic approach to healthcare; addressing the needs of the resident through mind, body and spirit. Our goal is to minister not only to the resident, but to their families, and the community.

Meadowbrook Care Center will deliver Exceptional Qualityof care and quality of life to our residents. We will provide a pleasant, supportive environment in which to live and work; and tailor our services to our communities' needs.
We shall pursue excellence in all performance disciplines and departments. Our center will be of Exceptional Serviceand assistance to our residents in fulfilling their physical, spiritual and emotional needs.

Meadowbrook has used NO agency staff since 2004.
The average tenure of our 200 staff members is amazingly over 5 years.
Meadowbrook is staffed with RNs 24/7 and 75% of our LPNs are IV certified.
We have a full time Certified Wound Care nurse and have ZER0 (0) in-house acquired pressure ulcers.

Weight loss in residents is less than half the state average.
Meadowbrook accepts Wound Vacs and Trachs.
We are at the top of the list when comparing local Skilled Nursing Facilitys Ohio Department of Health annual survey results demonstrated by our four-out-of-five star CMS Health Inspection rating.
Meadowbrook has an automated in-house pharmacy which allows for filling medication orders any hour of the day.
Meadowbrooks Activity department goes way beyond bingo, offering activities 7 days a week, including evenings. A large volunteer program ranging from youth to seniors supports our talented activity staff.
We are known for our Grand Sunday Brunches, Annual Easter Egg Hunt and Pumpkin Harvest Festival.
We DO have private rooms.

Physical, Occupational, and Speech Rehabilitation is provided 7 days a week, AND out-patient therapy is available.
Meadowbrook Care Center has a full time therapy team of seven (7) dedicated Physical, Occupational, and Speech therapy members with 150 years of combined experience and who have practiced at Meadowbrook for over 5 years together.
Our therapist goes to a residents home to do an assessment prior to discharge from Meadowbrook to ensure a successful transition to their home environment.

Our leadership team, including the Administrator and DON, have been together for over 7 years.
We have a commitment to service, offering a full time Chaplain to minister to the emotional and spiritual needs of residents, families, and staff.
Meadowbrook provides a Licensed Social Worker in each unit.
Our Social Workers coordinate a Care Conference with residents, families, and Staff within 72 hours of admission to discuss goals, expectations, and help with the transition to our facility.
We embrace the Eden Alternative concept by incorporating nature and pet therapy into the lives of our residents.
