We advise you to check with your current medical health insurance carrier to assure that we are in their
network of providers.
If you need FMLA/disability forms completed for your upcoming surgery or delivery, there will be a
$25 charge. Please allow us 7-10 business days to process your paperwork. The reception desk can
help you with your request.
Vitamin D is much more than a bone vitamin. It is crucial to many of our tissues and organs. Some of
these include the brain, breast tissue, colon, heart and lungs as well as our immune cells. There are
three ways to get Vitamin D: sunlight, foods (fortified milk, oily fish (tuna, salmon, etc)), or with daily
supplements. Daily recommendations are from 800-1000 units daily. Five to ten minutes of direct
sunlight between 10AM & 3PM will supply 3000 units which is stored in the body and released over time.

Osteoporosis is a prominent problem in Womens health! A Dexa Scan test is an easy and painless way
to diagnose this problem. The presence of certain risk factors may allow this procedure to be covered by
your insurance carrier We would strongly suggest that you check with your health care insurer to be certain that this is a covered procedure. Please feel free to contact our office to schedule a Dexa Scan test, or feel
free to discuss it with your provider on your next visit.
Our state of the art ultrasound machine continues to serve our patients very well. The use of this machine allows for excellent quality for our obstetric patients.
Our Gardasil HPV vaccine program continues with increasing interest. The vaccine is administered in a series of 3 injections. It is appropriate for those females from 9 26 years of age. It is the most recent clinical step toward protection from genital warts and cervical cancer. For additional information and guidelines, please contact our office or ask your provider for further information. Again, not all carriers cover this treatment, so please check prior to scheduling this procedure to be sure it is a covered treatment.
